Job Title: Home Health Physical Therapist

BAYADA Home Health Care is seeking a skilled Home Health Physical Therapist to join our team in Silver Spring, Maryland.

About the Job

This is a full-time opportunity to work as a Physical Therapist in a home health setting, providing rehabilitative care and skilled nursing to clients in their homes. As a Home Health Physical Therapist, you will be an integral member of a multi-disciplinary health care team, working closely with nurses and other healthcare professionals to develop and implement individualized care plans for our clients.

Responsibilities
  • Make home visits to clients in designated geographic territories to provide physical therapy services.
  • Perform diagnostic tests and measurements to assess clients' mobility, strength, and balance.
  • Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
  • Continually assess and revise care plans, participating in multidisciplinary care planning as needed.
  • Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations related to client care management and services provided.
Requirements
  • A current MD Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
