Job Title: Full-Time or Part-Time Home Care RNs, Registered Nurse

Job Summary:

We are seeking experienced Full-Time or Part-Time Home Care RNs to join our team in Prince George, Montgomery, and Howard County. As a Home Care RN, you will provide primary nursing care to patients in their homes, working collaboratively with our Population Health Case Managers to develop and implement care plans.

Responsibilities:

  • Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
  • Collaborate with Population Health Case Managers to develop and implement care plans, ensuring patient-centered care and optimal outcomes.
  • Provide functional support and supervision to Home Healthcare Aides and LPNs as needed.
  • Utilize motivational interviewing and health coaching techniques to achieve desired patient outcomes.
  • Adjust patient visit schedules to achieve care consistency, visit utilization, and operational efficiencies.
  • Initiate, maintain, and document communication regarding patient care and needs with physicians, clinical managers, other disciplines, and family members, as appropriate.

Requirements:

  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one (1) year professional nursing experience, preferably in Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit.
  • Strong computer and technology skills required.
  • Current Driver's license and reliable transportation.
