About the Job

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Job Summary

The Home Health LPN will be responsible for providing professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations. This includes providing skilled nursing interventions, teaching and training activities, and collaborating with the healthcare team to achieve individualized patient goals and outcomes.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN
  • Implement the nursing plan of care, utilizing a systematic and individualized approach
  • Confer with the RN Supervisor regarding needed changes in the Plan of Treatment
  • Accept verbal orders from physicians, as permitted by state law and regulations
  • Maintain contact with patients, physicians, and other members of the healthcare team regarding patient needs and status changes
  • Participate in care coordination activities and discharge planning, as appropriate
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
  • Current nursing license in the practicing state
  • Valid driver's license, auto insurance, and reliable transportation
  • Current CPR certification
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
