Job Overview

We are seeking a skilled Registered Nurse to join our team as a Total Joint Arthroplasty Nurse Navigator at Yale New Haven Health. This is a challenging yet rewarding role that requires strong clinical expertise, excellent communication skills, and a passion for patient-centered care.

Responsibilities
  • Work collaboratively with Service line leadership to identify high-risk patients for elective primary or revision TKA/THA surgeries and coordinate enrollment into risk modification programs.
  • Optimize patients' health needs through effective communication and resource allocation to promote quality, patient satisfaction, and cost-effective outcomes.
  • Carefully assess patients' medical history, condition, and psychosocial support to develop comprehensive plans of care.
  • Facilitate seamless transitions of care by collaborating with inpatient and outpatient therapists, primary care providers, and surgeons.
Qualifications
  • Bachelor's Degree in Nursing required; Master's degree in Nursing preferred.
  • Three to five years of nursing experience in Orthopaedic nursing, with a focus on clinical care, process improvement, outcomes management, case management, and physician relations.
  • Ability to thrive in a fast-paced academic medical environment and collaborate effectively with multidisciplinary teams.
Salary and Benefits

The estimated salary for this position is $85,000 - $110,000 per year, depending on experience. Our comprehensive benefits package includes health insurance, retirement plan, paid time off, and opportunities for professional growth and development.
