Case Manager I Job Overview

Position Summary:

The Case Manager I (CM-I) plays a vital role in delivering personalized case management services to individuals and families who have experienced homelessness. By employing a framework that includes Motivational Interviewing (MI), Critical Time Intervention (CTI), and Stages of Change (SoC), the CM-I encourages clients to pursue their objectives in a supportive and non-judgmental environment. Key responsibilities encompass intake procedures, comprehensive assessments, goal formulation, ongoing monitoring, life skills training, counseling, referrals, and data management within the Homeless Management Information System.

Core Responsibilities:

  • Deliver individualized case management and supportive services
  • Facilitate group support sessions
  • Maintain accurate records in a digital database
  • Process housing applications and conduct interviews with prospective tenants
  • Provide crisis intervention services and assist with tenant housing stability
  • Engage in evening and weekend activities
  • Report any concerns or instances of abuse
  • Regularly present tenant case evaluations

Qualifications:

Essential Qualifications:

  • Bachelor's degree in a Social Services discipline
  • Minimum of two years' experience in case management or client advocacy
  • Familiarity with client tracking database systems
  • Experience applying evidence-based practices in social services
  • Valid driver's license in California
  • Access to a personal vehicle
  • Compliance with insurance requirements

Preferred Qualifications:

  • Bilingual in English and Spanish
  • Experience in permanent supportive housing environments
  • Ability to facilitate life skills training groups
  • Proficient in the HMIS system
  • Knowledge of the Coordinated Entry System and VI-SPDAT
  • Strong crisis management capabilities
  • Proficient in Microsoft Office applications
  • Excellent communication skills
  • Experience working in a collaborative team setting
