Our initiatives encompass various criminal justice touchpoints, including pre-arrest interventions, court-based diversions, jail outreach, and intensive post-release support.

Program Overview
The Pretrial Release Evaluation Program (PREP) is a County-managed initiative designed to facilitate the release of criminal defendants from the county jail system at the arraignment phase. This program offers in-court evaluations, connections to treatment services, dedicated housing, and case management assistance for participants while their legal matters are pending.

Role Summary
Under the guidance of the Program Supervisor, the Client Support Specialist is tasked with delivering comprehensive, field-based case management and rehabilitative services to a designated group of pretrial clients. This role involves fostering and sustaining collaborative partnerships with external treatment and housing providers. A successful Client Support Specialist will engage clients actively in services, aiding them in achieving meaningful recovery objectives and enhancing their use of community resources.

Key Responsibilities
Deliver comprehensive, field-based support to promote recovery objectives
  • Engage clients in services by building strong relationships, maintaining consistent contact, and employing motivational interviewing techniques. Provide outreach services as necessary, meeting clients in their preferred environments.
  • Focus on the fundamental principles of recovery-oriented care, such as instilling hope, fostering empowerment, promoting self-responsibility, and encouraging meaningful participation. Ensure that service delivery aligns with ethical standards and a trauma-informed, strength-based approach.
  • Maintain an up-to-date understanding of available community resources. Conduct needs assessments and connect clients to appropriate services. Monitor ongoing engagement in services and assist in overcoming barriers. Accompany clients to appointments when suitable.
  • Teach essential recovery and social skills (e.g., communication, problem-solving, anger management) to support long-term community stability.
  • Collaborate with family members, probation officers, and court representatives as appropriate. Provide education on mental health recovery to better assist clients. Advocate for client needs.
  • Facilitate custody pickups from local jails.
Coordinate care with a multidisciplinary team
  • Contribute actively to the planning and implementation of interventions and referrals. Utilize the unique skills of team members to enhance service delivery.
  • Work in partnership with external organizations (e.g., residential treatment facilities, primary care clinics) to ensure high-quality, integrated care.
Documentation and Reporting
  • Maintain detailed case notes in the electronic health record in accordance with program guidelines.
  • Participate in program data collection as required.
Meet baseline expectations for Project 180 staff
  • Regular attendance is essential. Flexibility for holiday or weekend work may be necessary.
  • Adhere to the Agency's mission statement, values, policies, procedures, and principles.

Minimum Qualifications
Required:
  • Experience working with culturally diverse populations.
  • Valid driver's license, proof of insurance, and access to a vehicle required. Ability to transport clients as needed.
  • Must not be under any form of legal supervision.
  • Verification of Employment Eligibility and Background Check.
  • Basic computer proficiency.
  • Client Support Specialist I: Less than 2 years of case management experience.
  • Client Support Specialist II: 2 years of case management experience.
  • Client Support Specialist III: 3 or more years of case management experience.
Preferred Qualifications
  • Previous experience in case management.
  • Personal experience with homelessness, mental health or substance use recovery, or the criminal justice system.
  • Proficiency in Spanish.
  • Experience working with forensic and/or dual-diagnosed populations.
  • Experience in an interdisciplinary team within a mental health or hospital setting.

Supervisory Responsibilities

Work Environment
Job responsibilities will be carried out in both office and field settings.

Physical Requirements
The employee is expected to navigate the worksite throughout the day, which may involve walking, standing, climbing stairs, or using mobility aids. The employee must be able to drive, carry up to 20 lbs, listen, and communicate effectively to fulfill job responsibilities.

Mental Requirements
