Job Summary: As a Hospice Registered Nurse at Eden Health, you will provide high-quality, patient-centered care to individuals facing terminal illness in the comfort of their own homes. Our team is dedicated to delivering compassionate, skilled nursing services that address the physical, emotional, and spiritual needs of our patients and their families.

Key Responsibilities:
• Provide skilled nursing services to patients in their homes, including pain management, symptom control, and wound care.
• Collaborate with interdisciplinary teams to develop and implement individualized plans of care that meet the unique needs of each patient.
• Educate patients and their families on their rights and responsibilities as hospice patients, including end-of-life care and bereavement support.
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination and transitions.

Requirements:
• Current RN license in the state of practice.
• At least one year of experience in an acute-care setting, home health care, community health, or public health within the last 24 months.
• Valid driver's license and reliable transportation.
• Proof of current CPR certification and up-to-date COVID vaccine or exemption request.
