Job Summary

This is a full-time opportunity for a skilled RN to lead our hospice case management team. You will be responsible for developing trusting relationships with patients and their families, assessing their physical, psychosocial, and environmental needs, and coordinating their plan of care.

Responsibilities:
  • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral
  • Identify and re-assess patient and family needs no less than every 15 days
  • Collaborate with interdisciplinary teams to provide patient-centered care
Requirements

To be considered for this role, you must have:

  • One year of clinical experience as an RN
  • Current licensure in the state of practice
  • Current CPR certification
  • A valid driver's license and access to a reliable vehicle or public transportation
Salary and Benefits

The estimated salary range for this position is $75,000 - $90,000 per year, depending on experience. We also offer a comprehensive benefits package, including health insurance, retirement savings plan, and paid time off.
