Job Title: Practical Nurse in Hospice Care

Estimated Salary: $70,000 - $90,000 per year based on experience.

About the Job:

We are seeking an experienced Licensed Practical Nurse (LPN) to join our hospice care team. The ideal candidate will have a passion for delivering high-quality patient care and working collaboratively with healthcare professionals to improve patient outcomes. In this role, you will work closely with our interdisciplinary team to develop and implement personalized care plans that address the physical, emotional, and spiritual needs of patients and their families.

Key Responsibilities:

* Provide compassionate and individualized care to patients and their families, prioritizing their needs and preferences.
* Work collaboratively with registered nurses, social workers, chaplains, and other healthcare professionals to coordinate patient care and services.
* Develop and implement plans of care that address the unique needs of each patient, including symptom management, pain control, and emotional support.
* Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
* Participate in ongoing education and training to enhance knowledge and skills in palliative care and hospice service delivery.

Requirements:

* Completion of an accredited practical nurse or vocational nursing program.
* At least three years of nursing experience, preferably in hospice care or a related field.
* Current licensure as an LPN/LVN in the state.
* Demonstrated ability to work effectively in a fast-paced environment, prioritizing tasks and managing competing demands.
* Excellent communication and interpersonal skills, with a focus on empathy and cultural sensitivity.
