We are seeking a dedicated Registered Dietitian to work across our advanced skilled nursing facilities. This role requires adaptability to travel between various locations, providing support to the dietetic teams within each facility. The position encompasses administrative, scientific, and educational responsibilities aimed at delivering exceptional resident care through effective clinical nutrition management and tailored food service that meets individual resident needs. The dietitian will report directly to the facility Administrator while maintaining close collaboration with the corporate dietitian.

KEY RESPONSIBILITIES:
  • Conducts Clinical Assessments, revises patient care plans, and completes relevant MDS sections.
  • Maintains comprehensive knowledge of the Medicare PDPM system to ensure all patient needs are adequately addressed.
  • Documents within the facility's electronic medical record (PointClickCare) and utilizes meal planning software to ensure dietary needs are met.
  • Collaborates with the culinary team and dietary department to enhance food service operations.
  • Participates in meal services, actively addressing and resolving meal service challenges. Conducts meal rounds to monitor resident meal intake and assists with food service training as necessary.
  • Works with the facility to manage dietary costs by balancing nutritional care with the use of supplements and other interventions, monitoring supplement usage effectively.
  • Engages with patients, families, physicians, and the interdisciplinary team to promote Diet Liberalization, encouraging self-directed nutritional care for long-term care residents.
  • Communicates nutritional recommendations, weight changes, and other pertinent information to physicians and other disciplines, ensuring effective problem resolution.
  • Maintains a professional leadership role in the field of nutrition, addressing dietary-related issues, and collaborating with the food service manager to resolve food service challenges. Demonstrates strong time management skills and reliable attendance, functioning independently while fostering teamwork.
QUALIFICATIONS:
  • Must hold a valid license in the state of practice.
  • Detail-oriented individual who ensures timely completion of processes.
