We are seeking a skilled Registered Dietitian to provide exceptional nutritional care within our advanced skilled nursing facilities. This role requires adaptability to travel between various locations, ensuring comprehensive support to the dietary teams. The position encompasses administrative, scientific, and educational responsibilities aimed at delivering optimal resident care through effective clinical nutrition management and tailored food service solutions. The dietitian will report directly to the facility Administrator while maintaining close collaboration with the corporate dietitian.

KEY RESPONSIBILITIES:
  • Conducts Clinical Assessments, revises patient care plans, and completes relevant MDS sections.
  • Maintains proficiency in the Medicare PDPM system to ensure all patient needs are adequately addressed.
  • Documents in the electronic medical record (PointClickCare) and utilizes meal planning software to meet dietary requirements.
  • Collaborates with the culinary team and dietary department to enhance food service operations.
  • Participates in meal services, actively addressing and resolving any meal service challenges. Conducts meal rounds to monitor resident intake and assists with food service training as necessary.
  • Works with the facility to manage dietary costs by balancing nutritional care with supplement usage and other interventions.
  • Engages with patients, families, physicians, and the interdisciplinary team to promote Diet Liberalization, empowering long-term care residents with self-directed nutritional choices.
  • Communicates nutritional recommendations and weight changes to physicians and other disciplines, ensuring effective problem resolution.
  • Exhibits professional leadership in the field of nutrition, effectively addressing dietary challenges and collaborating with the food service manager.
  • Demonstrates strong time management skills and reliable attendance, functioning independently while fostering teamwork.
QUALIFICATIONS:
  • Must hold a valid license in the state of practice.
  • Detail-oriented individual committed to timely process completion.
