We are seeking a dedicated Registered Dietitian to provide exceptional nutritional care within our advanced skilled nursing facilities. This role involves flexibility to support various locations, ensuring optimal dietary management across our facilities. The position encompasses administrative, scientific, and educational responsibilities aimed at delivering the highest standard of resident care through tailored clinical nutrition strategies and food service solutions. The dietitian will report directly to the facility Administrator while maintaining close collaboration with the corporate dietitian.

KEY RESPONSIBILITIES:
  • Conducts comprehensive clinical assessments, updates patient care plans, and completes relevant MDS sections.
  • Maintains proficiency in the Medicare PDPM system to ensure comprehensive patient care.
  • Documents in the electronic medical record system and utilizes meal planning software to address patient dietary needs.
  • Collaborates with the culinary team and dietary department to enhance food service operations.
  • Participates in meal services, addressing any service-related issues, and monitors resident meal intake through meal rounds.
  • Assists in maintaining dietary cost control while balancing nutritional care and supplement usage.
  • Engages with patients, families, and healthcare teams to promote self-directed nutritional care.
  • Communicates nutritional recommendations and weight changes to physicians and other relevant parties.
  • Exhibits leadership in the field of nutrition, effectively addressing dietary challenges and collaborating with the food service manager.
QUALIFICATIONS:
  • Must hold a valid dietitian license in the practicing state.
  • Detail-oriented individual committed to timely completion of processes.
