Job Summary

We are seeking a skilled Care Manager to join our team at Duke Health. As a Care Manager, you will play a critical role in providing high-quality patient care and support to patients and their families in their homes and communities.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to ensure optimal health outcomes.
  • Provide education and support to patients and families on disease management, medication adherence, and self-care.
  • Collaborate with healthcare teams to coordinate care and services, including home health, hospice, and infusion services.
  • Conduct regular patient assessments and evaluations to ensure quality of care and make adjustments to care plans as needed.
  • Document patient information and care plans in accordance with Duke Health policies and procedures.
Requirements
  • Associate Degree or Diploma in Nursing required; BSN preferred.
  • Current North Carolina RN licensure or compact RN licensure in participating state.
  • 2+ years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
