About the Role

We are seeking a highly skilled and dedicated Population Health Care Manager to join our team at Duke Health. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and improving health outcomes for our patients.

Key Responsibilities
  • Develop and Implement Care Plans: Create and implement comprehensive care plans that address the unique needs of our patients, including those with chronic health conditions and complex care requirements.
  • Coordinate Care Transitions: Ensure seamless transitions of care between healthcare settings, including hospitals, clinics, and community-based care.
  • Provide Education and Support: Educate patients and their families on healthy behaviors, disease management, and self-care techniques to promote optimal health outcomes.
  • Collaborate with Healthcare Teams: Work closely with healthcare providers, social workers, and other interdisciplinary team members to ensure comprehensive and coordinated care.
  • Monitor and Evaluate Care: Continuously monitor and evaluate the effectiveness of care plans and make adjustments as needed to ensure optimal patient outcomes.
Requirements
  • Education: Bachelor's degree in Nursing or Master's degree in a related field (e.g., counseling, social work, allied/behavioral health).
  • Experience: 3 years of relevant clinical experience required.
  • Certifications: Current licensure as a Registered Nurse (RN) or a related healthcare professional, and a case management certification (ACM, CCM, or ANCC) within 3 years of hire.
