RN Patient Care Manager

We are seeking a highly skilled RN Patient Care Manager to join our team at SunCrest Home Health, a part of LHC Group. As a Patient Care Manager, you will play a vital role in ensuring the delivery of high-quality, patient-centered care in the comfort of patients' homes.
Key Responsibilities:- Supervise and coordinate clinical services to ensure seamless patient care
- Provide clinical guidance and support to patient-facing teams
- Ensure compliance with applicable laws, regulations, and agency policies
- Collaborate with the healthcare team to develop and implement individualized care plans
- Conduct regular patient assessments to evaluate care needs and effectiveness
- Ensure timely and accurate documentation of patient information
- Current RN licensure in the state of practice
- Current CPR certification
- Current Driver's License and access to a dependable vehicle or public transportation
- Strong leadership and communication skills
- Ability to work independently and as part of a team
