Job Title: Population Health Care Manager

Job Summary:

The Population Health Care Manager is a critical role in our organization, responsible for delivering clinical expertise to manage the healthcare needs of specific patient populations. This position functions as an integral part of an interdisciplinary team, working to optimize clinical outcomes through seamless transitions, access, and care.

Key Responsibilities:

  • Develop and implement care plans to address the unique needs of patient populations
  • Collaborate with healthcare providers to ensure timely and effective care
  • Conduct thorough assessments to identify patient needs and develop targeted interventions
  • Provide education and support to patients and families to promote self-management and healthy behaviors
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed

Requirements:

  • Bachelor's degree in Nursing or Master's degree in a related field
  • 3 years of relevant clinical experience
  • Current licensure as a Registered Nurse (RN) or other relevant healthcare professional
  • Case management certification (ACM, CCM, or ANCC)
