Position Overview

As a RN Care Coordinator within our Surgical Epilepsy Center, you will be an integral part of a multidisciplinary team dedicated to the comprehensive care of patients with epilepsy. This role is based at our Main Campus, where you will collaborate with healthcare professionals to ensure seamless care delivery for high-risk patients.

Key Responsibilities:
  • Identify patients requiring ongoing care coordination for their specialty conditions.
  • Assess the involvement needed from the specialty care team and coordinate with primary care providers.
  • Utilize assessment tools to determine patients' care needs and conduct targeted outreach to high-risk individuals.
  • Manage patient populations using technological tools and conduct thorough clinical assessments.
  • Collaborate with the healthcare team to develop care plans aimed at optimizing patient outcomes.
  • Monitor patient adherence to care plans and make necessary adjustments based on progress.
  • Serve as the primary contact for patients and families, advocating for their needs and addressing concerns.
  • Facilitate transitions of care and work to overcome barriers to receiving services.
  • Engage with other care coordinator teams to ensure comprehensive patient support.

Qualifications:
  • Current state licensure as a Registered Nurse (RN).
  • Experience in nursing, preferably with a focus on chronic disease management.
  • Strong communication and critical thinking skills.
  • Ability to work both independently and collaboratively within a team.
