Job Overview

The Practical Nurse Specialist is responsible for the assessment and coordination of care for patients participating in the Screening & Prevention program.

Key Responsibilities
  • Manage incoming orders, conduct thorough chart reviews, triage patient inquiries, and perform phone assessments to evaluate patient eligibility based on established screening criteria.
  • Act as a communication bridge for the department, liaising with referring offices, physicians, and medical facilities regarding patient eligibility, concerns, and necessary follow-up actions.
  • Provide clinical guidance to patients and healthcare providers about procedures, preparation instructions, and eligibility requirements.
  • Review and clarify preparation instructions with patients, and submit orders for procedural preparations under the supervision of physician management.
  • Interpret and communicate results from scheduled colonoscopy procedures, ensuring that findings are documented and conveyed to both patients and referring providers.
  • Administer treatments as prescribed by healthcare providers and respond promptly to patient prescription requests and inquiries.
  • Oversee patient recall for follow-up colonoscopies and adhere to directives from the performing physician.
  • Accurately document all observations, interventions, and evaluations related to patient care management using electronic medical records on advanced touch pad tablets.
  • Ensure proper labeling and provision of insurance information for referrals to labs, x-rays, or specialists, adhering to insurance protocols while maintaining accuracy in coding and billing.
  • Assist in patient education and discharge planning to promote optimal recovery and understanding of care.
  • Maintain up-to-date knowledge, certification, and licensure relevant to the role.
  • Commit to delivering high-quality service and patient care at all times.
Qualifications

Minimum Education
  • Technical Diploma/Certificate in Practical Nursing is required.
Licenses and Certifications
  • Must possess a Licensed Practical Nurse license in Florida or an eligible compact state license upon hire.
  • BLS for Healthcare Providers certification from the American Heart Association is required upon hire.
Required Skills, Knowledge, and Abilities
  • Exceptional verbal and written communication skills (High proficiency).
  • Thorough understanding of patient care charts and histories (High proficiency).
  • Expertise in patient evaluation and triage processes (High proficiency).
  • Knowledge of appropriate medication administration procedures and patient care standards (High proficiency).
  • Familiarity with clinical and surgical operations and documentation practices (High proficiency).
  • Understanding of supplies, equipment, and inventory management (High proficiency).
  • Ability to read and comprehend the English language effectively (High proficiency).
