Job Summary

We are seeking a highly skilled Service Access Coordination Manager to join our team at Coordinated Care Services, Inc. (CCSI). This is a full-time exempt position that requires a strong background in mental health, social work, or a related field.

Key Responsibilities
  • Program Oversight: Provide leadership and oversight to the Rapid Engagement Delivery (RED) Program, Safe Options Support (SOS) Critical Time Intervention (CTI) HUB Coordinator, and SAMHSA Homeless Partnership Project.
  • Collaboration and Partnerships: Foster and develop collaborative relationships with community partners to identify and address needs and barriers in the homeless/at risk of homelessness community.
  • Case Management: Provide case management and conduct home visits as appropriate.
  • Grant Management: Manage pool of funding to purchase short-term stays in various residential settings for the SAMHSA project.
  • Evaluation and Improvement: Evaluate program effectiveness, develop improved methods, and implement changes to ensure effective sustainability of programs and funding.
  • Communication and Representation: Conduct presentations on program/project to community agencies and stakeholders, and act as a representative of CCSI and the Office of Mental Health in key stakeholder meetings.
  • Supervision and Development: Supervise and professionally develop staff members.
Requirements
  • Education and Experience: Bachelor's degree in mental health, social work, or a related field, and 5 years' experience in the CD/MH field, or equivalent education and work experience.
  • Technical Skills: Experience with Windows-based operating system, including word processing, spreadsheet, and database management software.
