Full-Time Position

Coordinated Care Services, Inc. (CCSI) is in search of a dedicated Access Coordination Services Manager to enhance our essential business functions for our partner organizations and clients. This role entails overseeing and monitoring various programs aimed at providing support to individuals experiencing homelessness or at risk of homelessness.

The Access Coordination Services Manager will work collaboratively with community stakeholders to identify and mitigate challenges faced by vulnerable populations. This position is crucial in fostering partnerships with local agencies to address issues related to homelessness, social determinants of health, and access to mental health or substance use disorder services.

Key Responsibilities:

  • Oversee daily operations and provide technical assistance for multiple programs, including leading meetings, facilitating quality improvement initiatives, and addressing ongoing concerns from clients and community partners.
  • Conduct case management and home visits as necessary.
  • Engage in case reviews and coordinate with the care continuum.
  • Develop operational policies, procedures, and workflows; manage training schedules and ensure compliance with established plans.
  • Administer funding for short-term residential placements as part of designated projects.
  • Assess program effectiveness and implement improvements; analyze data and recommend actions for sustainability.
  • Deliver presentations on programs to community agencies and stakeholders.
  • Supervise and mentor team members to promote professional growth.
  • Address inquiries regarding referral processes and project services.
  • Participate in ongoing improvement initiatives as directed.
  • Facilitate communication among service providers and within the community.
  • Recognize and respect the diverse backgrounds of clients in all interactions.
  • Maintain confidentiality regarding sensitive information encountered during work.
  • Review and update processes and policies as necessary.
  • Represent CCSI and relevant stakeholders in key meetings.
  • Travel as required to fulfill job duties.
  • Ensure timely reporting and data submission for all funded projects.
  • Manage grant budgets and oversee renewals and sustainability efforts.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's degree in mental health, social work, or a related field, with a minimum of five years of experience in the mental health or substance use disorder sector, or equivalent education and experience.
  • Proficiency in using Windows-based software for word processing, spreadsheets, and database management.

  • Successful completion of all necessary background checks.
  • Adhere to confidentiality protocols regarding client information and comply with CCSI's privacy policies and applicable regulations.
