Overview of the Role

MedStar Health is seeking a qualified Speech Language Pathologist on a per visit basis to provide exceptional care in home settings. The ideal candidate will possess a valid license in Speech Language Pathology and demonstrate outstanding clinical and interpersonal skills.

The Speech Language Pathologist will conduct assessments and deliver speech-language therapy in compliance with agency protocols, as well as state and federal regulations governing speech-language pathology services.

Key Responsibilities:


Coordinate specialized assessments that are not feasible in a home setting (e.g., audiological evaluations, video fluoroscopy studies) and ensure proper discharge planning, including referrals to suitable community resources (e.g., outpatient therapy, daycare) that align with client needs post-discharge.

Maintain regular communication with supervisors regarding the delivery of rehabilitation services and individual patient care concerns.

Foster collaborative relationships with other departments and engage in multidisciplinary quality improvement initiatives. Participate in meetings and represent the department in community outreach programs. Enhance professional growth and knowledge through educational opportunities and affiliations.
Maintain ongoing dialogue with the patient's physician, healthcare team, and other relevant parties (e.g., insurance case managers, referring facility staff) concerning care aspects and patient progress.

Conduct assessments of the patient's living environment and recommend adaptive equipment or home modifications to optimize functional abilities and safety.

Assist in acquiring items eligible for reimbursement as durable medical equipment. Perform initial and ongoing assessments to identify the patient's rehabilitation needs.

Develop a comprehensive speech-language pathology treatment plan in collaboration with the patient, their family/caregiver, physician, and other healthcare team members.

Adjust the treatment plan as necessary. Evaluations will include interviews, observations, palpation, and specialized assessments such as dysphagia evaluations, oral motor examinations, and language assessments.

These assessments require specific tools (e.g., sphygmomanometer, stethoscope) and the ability to utilize sight and hearing senses effectively.

Document medical records in accordance with agency standards, professional guidelines, and regulatory requirements for speech-language pathology service reimbursement.

Provide education to patients and/or caregivers regarding the injury/disease process, functional limitations, and treatment interventions, along with training in care management post-discharge.

Deliver speech-language pathology services in alignment with the treatment plan, agency standards, and other professional and regulatory guidelines. Implement procedures and modalities, including associative language exercises, oral motor exercises, aural rehabilitation, and thermal stimulation. Supervise home health aides in relevant patient care activities.

Qualifications:

Master's degree in Speech-Language Pathology.
1-2 years of clinical experience.

License to practice Speech-Language Pathology in the relevant state, or Certificate of Competency from ASHA (American Speech-Language-Hearing Association) if applicable.

CPR certification.
Valid State Driver's License in good standing.
