Job Title: Patient Service Coordinator

Become a part of our caring community and help us put health first. As a Patient Service Coordinator, you will play a vital role in ensuring the continuity of care for our patients. Your responsibilities will include scheduling visits, communicating with field staff, patients, and physicians to maintain proper care coordination.

Key Responsibilities:
  • Schedule visits and manage patient care coordination
  • Communicate with field staff, patients, and physicians to ensure seamless care delivery
  • Assist with day-to-day office and staff management
  • Manage schedules for all patients and ensure timely reassignment of visits
  • Initiate infection control forms as needed and send completed reports to HRD
  • Provide backup support during busy times, including receiving calls from field staff and assisting with weekly case conferences
  • Refer clinical questions to Branch Director as necessary
  • Maintain client hospitalization logs and send electronic logs to relevant staff
  • Complete requested schedules as tasks appear on the action screen
  • Ensure staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction
  • Process reassigned and rescheduled visits
  • Run scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report
  • Prepare weekly Agent Schedules and perform initial review for productivity/geographic issues
Requirements:
  • Excellent communication skills and ability to interact with diverse individuals
  • Strong organizational skills and ability to manage multiple assignments
  • At least 1 year of home health experience
  • Prior packet review/QI experience preferred
  • Coding certification is preferred
