Nursing Enrollment Specialist

The Enrollment RN is crucial to this mission, facilitating the eligibility process and ensuring seniors can access our program.
The Enrollment RN position emphasizes performing initial nursing and home safety evaluations of potential participants in their residences, while also overseeing the State level-of-care (LOC) review process in collaboration with a Department of Health Care Services nurse. As a member of the outreach and enrollment team, the Enrollment RN collaborates closely with physicians, social workers, and other team members.
Key Responsibilities:
- Conduct initial nursing evaluations, including nursing assessments, functional evaluations, and home safety checks, to ascertain if prospective WelbeHealth participants meet PACE eligibility criteria.
- Timely prepare documentation from home assessments, submit to the California State Level-of-Care (LOC) nurse for review, and address inquiries to expedite the review process for participants.
- Monitor and manage the annual State level-of-care review process.
- Collaborate with the interdisciplinary team (IDT) and outreach & enrollment (O&E) team.
- Ensure the privacy and confidentiality of all company procedures and information regarding team members, participants, and their families.
Qualifications:
- Graduation from an Accredited School of Nursing is required; a B.S. in Nursing is preferred.
- An unencumbered RN License in the state of practice is required.
- A minimum of two (2) years of nursing experience in a clinical environment with a frail or elderly demographic is preferred.
- Reliable transportation is necessary.

The salary range for this role is $57.29 hourly plus bonuses and equity. WelbeHealth offers a competitive total rewards package that includes a 401k match, healthcare coverage, and a wide array of additional benefits. Actual compensation will be adjusted based on experience and other qualifications.
