As a key member of our housing community, you will be responsible for providing supportive services to elderly and physically challenged residents, ensuring their needs are met and they can safely age in place.
The ideal candidate will have a strong background in social work, gerontology, or a related field, with excellent communication and interpersonal skills. You will work closely with residents, staff, and community-based providers to assess and address individual needs, develop and implement health-driven aging plans, and provide education and resources to promote healthy aging.
Key responsibilities will include:
- Providing case management and support to residents, including assessment of needs and development of individualized plans
- Collaborating with social services partners and healthcare providers to ensure coordinated care and support
- Managing the 504 reasonable accommodation process and ensuring timely completion
- Preparing and submitting reports, including semi-monthly management and monthly frail and at-risk reports
- Maintaining accurate and up-to-date resident information and data
