PROGRAM OVERVIEW
The Supportive Services for Veteran Families (SSVF) program focuses on case management for Veterans and their families who are homeless or at risk of homelessness. SSVF Case Managers are instrumental in identifying and overcoming barriers, enhancing self-sufficiency, and securing permanent housing solutions. We collaborate closely with the VA and other community organizations to ensure our clients have access to all available resources.

KNOWLEDGE REQUIREMENTS
- Understanding of SSVF core principles: Housing First, Crisis Response, and Client Choice.
- Familiarity with trauma-informed care practices.
- Experience with individuals facing homelessness or in crisis.
- Awareness of community resources available in the region.
- Knowledge of housing regulations and confidentiality practices.
RESPONSIBILITIES
- Manage a caseload of households within the SSVF Program, assessing housing needs and providing necessary support.
- Utilize Housing First and Motivational Interviewing techniques to engage with Veteran families.
- Develop Housing Stability Plans in collaboration with families and connect them with additional resources.
- Establish and maintain partnerships with community resources and participate in relevant meetings.
- Conduct outreach to identify homeless Veterans and promote the SSVF Program.
- Complete thorough intakes and eligibility assessments for referred clients.
- Document client interactions and maintain accurate case management records.
- Adhere to established policies and procedures of the SSVF Program.
- Conduct home visits to support clients in achieving housing stability.
- Perform other related duties as assigned.
QUALIFICATIONS
Minimum Education: High School Diploma or GED.
Minimum Experience: One year of experience working with individuals or families with complex needs, particularly in supportive services for Veterans.
Knowledge of trauma-informed care, motivational interviewing, and client-centered case management is essential. Candidates with military backgrounds or experience in serving Veterans are encouraged to apply.
WORK ENVIRONMENT
Work is primarily conducted in an office setting, with fieldwork required within the community. Occasional lifting of up to 25 pounds may be necessary, and the role involves sitting or standing for extended periods.
