Overview:
The organization is dedicated to supporting individuals with disabilities, behavioral challenges, complex medical needs, transitioning youth, the elderly, and other populations through comprehensive case management and care coordination models.

Role Summary:

The Community Resource Navigator plays a vital role in assisting individuals with disabilities and their families in identifying and accessing resources, fostering connections, and enhancing their independence and community involvement.

This position is essential for linking individuals to the necessary services and resources that cater to their unique needs, collaborating closely with service providers and community organizations to ensure that families receive the appropriate support and information.

The Navigator is also responsible for the design, coordination, and execution of community outreach and educational initiatives aimed at better serving both the individuals supported by the organization and those who remain unserved in the community.

Key Responsibilities:

Act as the primary contact for individuals seeking information and referral services through the organization’s resource line.

Establish rapport and conduct needs assessments within the context of family and community dynamics. Utilize frameworks to identify strengths, barriers, and choices. Provide education on available supports and services, making appropriate referrals based on individual preferences. Engage in community outreach by building relationships with service providers, community groups, and other organizations.

Assist individuals and families in navigating the application processes for Medicaid and Home and Community-Based Waiver Programs.

Document and track support systems, gathering and organizing information while assessing the effectiveness and satisfaction of referrals to enhance quality. Advocate for individuals with disabilities to promote independent living and ensure tailored support.

Develop and deliver training to families to empower them in supporting their loved ones with disabilities.

Collaborate with community organizations and events to maximize resources for families.

Research and create a comprehensive resource database that addresses all life domains, promoting integrated services for individuals to achieve their desired quality of life.

Foster opportunities for peer connections among families and individuals supported.

Connect individuals and families with advocacy resources to ensure their involvement in policy-making and service implementation.

Ensure timely responses to all incoming inquiries from individuals, families, and community members. Participate in ongoing training and professional development to stay updated on new resources and services.

Obtain relevant professional certifications in information and referral services as necessary.


Education and Experience:
A Bachelor’s degree in a relevant field (such as social work, public health, or psychology) is required.

Two years of experience in information and referral services, case management, or a related field is preferred. Knowledge of community resources and social services is essential. Proficiency in technology, including databases and Microsoft Office Suite, is required. Professional credentials in information and referral services are a plus.
