Job Title: Team Leader - Social Services

FLSA Status: Exempt

Department: Transitional Housing

Annual Salary: $63,672.44

Reports To: Program Director

Supervisory Role: Peer Coordinator

The Team Leader is responsible for delivering high-quality clinical services and mental health support to women facing mental health challenges in transitional housing. This role involves close collaboration with social service professionals to effectively address client needs and ensure optimal service delivery.

Key Responsibilities:

  • Facilitate the initial intake process in collaboration with security personnel.
  • Support clients in adapting to the transitional housing program.
  • Assess client needs and provide immediate service triage.
  • Conduct comprehensive psycho-social evaluations for assigned clients.
  • Deliver emergency and crisis intervention services.
  • Develop and adjust Relapse Prevention Plans in partnership with clients.
  • Ensure accurate and timely data entry into the Department of Homeless Services (DHS) database.
  • Lead three group sessions weekly and provide detailed reports to management.
  • Offer supportive counseling and training in Activities of Daily Living (ADL) to clients.
  • Availability to work evenings, at least one Saturday per month, and some holidays is required.
  • Establish and maintain an advocacy network with community resources.
  • Actively engage as a member of the shelter's multi-disciplinary team.
  • Participate in transitional housing meetings, organization-wide meetings, and required staff training.
  • Prepare mandated reports as required by supervisors and contract agencies.
  • Collaborate with the multi-disciplinary team to strategize treatment plans and crisis interventions.
  • Assist in planning and coordinating special events for clients.
  • Connect clients with substance abuse and mental health treatment programs.
  • Work with community-based service providers to ensure client engagement.
  • Advocate for clients to secure public and private benefits and services.
  • Collaborate with healthcare providers to ensure comprehensive treatment plans, including medication management.
  • Engage in other special projects and responsibilities as assigned.

Essential Functions:

  • Must be able to work evenings, at least one Saturday a month, and some holidays.
  • Ability to navigate stairs within the assigned building.
  • Capability to escort clients to appointments as necessary.

Qualifications:

  • Commitment to the mission, vision, and values of the organization.
  • LMSW required, with a minimum of one year of post-graduate experience.
  • Supervisory experience is advantageous.
  • Experience working with individuals facing mental health issues, substance abuse, and homelessness.
  • Proficient in Motivational Interviewing, Trauma-Informed Care, Strengths-Based Case Management, Harm Reduction, and Recovery-Oriented principles.
  • Proven ability to facilitate group treatment and develop curricula.
  • Familiarity with entitlement systems and procedures.
  • Strong organizational, written, and verbal communication skills.
  • Proficiency in Microsoft Office applications.
  • Bilingual proficiency in English and Spanish is a plus.
