Position Summary:
The Clinical Manager is tasked with ensuring that patient care is effectively coordinated and managed, alongside supervising the clinical team. This role guarantees that all home health services provided to newly admitted and active patients comply with state regulations and Medicare Conditions of Participation. The Clinical Manager collaborates with team members and nursing staff to ensure optimal care coordination for all active Home Health cases. Additionally, this position assists in designing and implementing metrics to monitor and enhance processes for effectiveness and quality, while upholding the mission, vision, and values of VNA Health.

Key Responsibilities:
Oversight of Interdisciplinary Team:
  • Evaluates referrals and patient information to assess home health needs, making patient and personnel assignments while collaborating with scheduling to assign suitable clinicians.
  • Coordinates patient care by reviewing services provided, guiding clinicians for improved performance, and being available during operating hours for support.
  • Monitors nursing and rehabilitation schedules, patient care, outcomes, and compliance with treatment orders.
  • Ensures timely processing of physician orders and reviews departmental reports for efficiency and quality of care.
  • Conducts weekly case management reviews with team members and monitors lab values for team patients.
  • Establishes workflows for telehealth patients and maintains communication protocols to facilitate team collaboration.
  • Resolves patient and physician complaints while adhering to relevant regulations and standards.
  • Prioritizes interventions to address issues related to care delivery and staffing productivity.
  • Participates in on-call duties as required, ensuring clinical management availability during all operating hours.
  • Engages actively in quality management initiatives.

Team Coordination:
  • Facilitates monthly coordination for assigned cases and leads team case conferences to ensure continuous assessment of patient needs.
  • Ensures completeness of admission documentation and coordinates referrals to appropriate disciplines.
  • Attends case conferences with clinical personnel to enhance care coordination.
  • Facilitates monthly team meetings to share updates and discuss operational issues.
  • Oversees bi-weekly payroll processes, ensuring compliance with approval protocols.

Personnel Management:
  • Participates in the recruitment and interviewing of new personnel, providing recommendations for hiring.
  • Oversees the orientation and competency assessments of new clinical staff, ensuring compliance with regulatory standards.
  • Creates a mentoring environment for clinical staff, evaluating effectiveness and areas for improvement.
  • Conducts performance evaluations for clinical staff and addresses disciplinary matters as needed.
The above responsibilities represent a summary of the primary duties and tasks associated with this role. Incumbents may be required to perform additional job-related tasks as necessary. All clinical staff are expected to participate in variable shifts, including weekend rotations and holiday schedules as needed.
Qualifications:
  • Registered Nurse with current licensure in good standing in the state of CA.
  • Bachelor's degree preferred.
  • Minimum of two years of management or supervisory experience in a healthcare setting, preferably in home care.
  • Current CPR certification.
  • Licensed driver with an insured vehicle, in accordance with state and/or organizational requirements.
  • Adheres to accepted professional standards and practices.
  • Exhibits strong observation, verbal, and written communication skills.
Physical Requirements:
  • Ability to lift and push/pull 50 lbs.
  • Ability to stoop, bend, and squat.
  • Ability to walk up one flight of stairs and one mile.
  • Ability to sit for at least one hour.
  • Visual and auditory acuity.
