Position Summary:
The Clinical Manager plays a crucial role in ensuring that patient care is effectively coordinated and managed while supervising the clinical team. This position guarantees that all home health services provided to both newly admitted and existing patients comply with state regulations and Medicare Conditions of Participation. The Clinical Manager collaborates closely with team members and nursing staff to ensure optimal care coordination for all active home health cases. Additionally, this role involves designing and implementing metrics to monitor and enhance process effectiveness and quality, while upholding the mission, vision, and values of VNA Health.

Key Responsibilities:
Oversight of Interdisciplinary Team:
  • Evaluates patient referrals and available information to determine home health needs, making assignments for patients and personnel while collaborating with scheduling to allocate appropriate clinicians.
  • Coordinates patient care by reviewing services provided, guiding clinicians for improved performance, and being available during operating hours for support.
  • Monitors schedules, patient care, outcomes, and interdisciplinary care to ensure compliance with treatment orders.
  • Ensures timely processing of physician orders and reviews departmental reports for efficiency and quality.
  • Conducts weekly case management reviews with team members and monitors lab values for patients, documenting findings in clinical records.
  • Establishes workflows for telehealth patients and maintains effective communication to facilitate team collaboration and adherence to best practices.
  • Addresses patient and physician complaints, ensuring compliance with relevant regulations and standards.
  • Manages staffing requirements and productivity guidelines while assigning patient visits based on staff competency.
  • Participates in on-call duties as defined by policy, ensuring clinical management availability during operating hours.
  • Engages actively in quality management initiatives.
  • Performs additional duties as assigned by the Director of Home Health.

Team Coordination:
  • Facilitates monthly coordination for assigned team cases and leads case conferences to ensure ongoing assessment of patient needs.
  • Ensures completeness of admission documentation and coordinates referrals to appropriate disciplines.
  • Participates in case conference meetings to enhance care coordination.
  • Conducts monthly team meetings to provide updates and discuss operational issues.
  • Oversees bi-weekly payroll, ensuring all overtime is approved by the Director.

Personnel Management:
  • Involved in the recruitment and selection process for new personnel, providing recommendations for hiring.
  • Oversees orientation and competency evaluations for new clinical staff, ensuring compliance with standards.
  • Creates a mentoring environment for clinical staff, evaluating effectiveness and suggesting improvements.
  • Conducts performance evaluations for employees, providing feedback and guidance for improvement.
  • Addresses employee performance issues in a timely and confidential manner, promoting conflict resolution and enhanced performance.
The above statements represent a summary of the primary duties and responsibilities of this role. Incumbents may be required to perform additional job-related tasks as necessary. All clinical staff are expected to participate in variable shifts, including weekend rotations as needed.
Qualifications:
  • Registered Nurse with current licensure in good standing in the state of CA.
  • Bachelor's degree preferred.
  • Minimum of two years of management or supervisory experience in a healthcare setting, preferably in home care.
  • Current CPR certification.
  • Licensed driver with insured vehicle as per state and organizational requirements.
  • Adheres to accepted professional standards and practices.
  • Exhibits strong observation, verbal, and written communication skills.
Physical Requirements:
  • Ability to lift and push/pull 50 lbs.
  • Ability to stoop, bend, and squat.
  • Ability to walk up one flight of stairs and one mile.
  • Ability to sit for a minimum of one hour.
  • Visual and auditory acuity.
