Job Summary:
The Patient Care Associate will play a vital role in supporting our clients with personal care and homemaker needs. This full-time position requires a Certified Nursing Assistant (CNA) certification and at least 2 years of experience in the healthcare field.

Key Responsibilities:
- Provide personal care and homemaker services to clients as per their individual care plans.
- Review and follow current care plans, documenting all care provided accurately and efficiently.
- Communicate regularly with supervisors and schedulers regarding availability and time-off requests.
- Attend departmental meetings and training sessions to enhance skills and knowledge.
- Report any changes or concerns about client functioning, health, or overall well-being to supervisors and nurses.

Requirements:
- Minimum 2-3 years of experience in the healthcare field.
- Certified Nursing Assistant (CNA) certification required.

About the Role:
