Patient Care Coordinator for Medical Teams

We are seeking a dedicated Patient Care Coordinator to join our team at Cone Health. As a vital member of our medical staff, you will work closely with patients and healthcare professionals to provide exceptional care and support.
About the RoleThis is a $45,000 - $55,000 per year opportunity that requires strong communication and organizational skills. You will be responsible for coordinating patient care, documenting information, and collaborating with registered nurses to ensure seamless delivery of services.
Key Responsibilities- Observe and report changes in patient symptoms and behaviors to the registered nurse
- Communicate effectively with patients and their families to understand needs and concerns
- Assist with patient care responsibilities, including vital signs, capillary blood glucose, and EKGs
- Facilitate patient educational groups and provide support as needed
To be successful in this role, you will need:
- A high school diploma or equivalent
- Completion of a state-approved Nurse Aide 1 training program
- No prior experience required, but preferred if you have 1 year of experience as a nursing assistant
