Job Overview

Position Purpose

The primary objective of this role is to perform comprehensive evaluations of participants in the PACE (Program of All-Inclusive Care for the Elderly) initiative, working in close collaboration with other members of the Interdisciplinary Team (IDT) to develop and execute personalized care strategies. This position is tasked with designing a wide array of activities and wellness programs aimed at improving the physical and emotional health of participants. Furthermore, the role encompasses collaborating with participants and their families to create and promote engaging recreational activities within their homes, thereby nurturing a supportive and enriching atmosphere that enhances participants' overall quality of life.

Key Responsibilities

  • Designs and organizes a variety of activities, including but not limited to, physical exercise, arts and crafts, celebrations, special events, recreational games, community outings, and cognitive engagement groups.
  • Coordinates community outings and special events at the Adult Day Health Center (ADHC), managing transportation, entertainment, catering, and necessary staff.
  • Creates and disseminates a monthly activities calendar.
  • Plans and executes small group or individualized activities tailored for participants with dementia, including sensory stimulation, while overseeing the daily operations of the dementia activities area.
  • Supports participants and caregivers by offering leisure education for activity adaptation or skill enhancement, along with supplies for home use as required.
  • Acts as a member of the interdisciplinary team; participates in team meetings and periodic evaluations, reporting any changes in participants' baseline conditions to the appropriate staff daily.
  • Maintains order in activity areas, including daily clean-up, routine inventory management, ordering, and purchasing, while overseeing overall budgeting.
  • Evaluates participant engagement in activities within group or nursing home settings, which may involve direct visits to these locations. Collaborates with nursing home and community-based living facility staff to positively influence participants in these environments.
  • Completes necessary documentation, including initial assessments, periodic reviews, individual service plans, and other participant data as directed by the interdisciplinary team.
  • Provides ongoing support to all staff involved in dementia-specific programming related to the dementia disease process.
  • Facilitates participant transportation within the ADHC and rearranges activity spaces as necessary.
  • Ensures compliance with all regulatory requirements, reimbursement standards, organizational policies, and procedures relevant to the area of responsibility.
  • Consistently upholds high standards of accuracy in work products and documentation.
  • Adheres to universal precaution procedures and proper safety techniques in line with established policies and procedures.
  • Safeguards the privacy and confidentiality of associates, participants, and sensitive agency information.
  • Performs other duties as assigned.

Qualifications

Education:

  • Bachelor's degree in Recreation Therapy or a closely related field is preferred.
  • Current BLS/First Aid certification is preferred.

Experience:

  • A minimum of two years of experience working with frail or elderly populations.

Skills and Knowledge:

  • Strong written and verbal communication skills.
  • Proven ability to problem-solve and exercise sound judgment.
  • Ability to work independently and effectively.
  • Competence in using or willingness to learn computer skills for workplace applications.
  • Demonstrates the necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

Other Requirements:

  • Physical strength to perform job duties.
  • Routine travel between PACE sites and within the community is required.
  • A valid driver's license is necessary.

Physical Demands:

  • Must be able to remain stationary for 50% of the time.
  • Ability to occasionally move about the office to access file cabinets and office machinery.
  • Capable of operating a computer and other office productivity equipment, such as calculators, copy machines, and printers.
  • Able to position oneself to maintain files in cabinets, which may involve reaching, kneeling, or crouching.
  • Effective communication and the ability to exchange information are essential for this role.
