Job Overview

Position Objective

The objective of this position is to perform comprehensive evaluations of PACE (Program of All-Inclusive Care for the Elderly) participants, working in close collaboration with other members of the Interdisciplinary Team (IDT) to develop and execute effective, personalized care strategies. This role is accountable for designing a variety of activities and exercise initiatives aimed at improving the physical and emotional health of participants. Furthermore, the position entails engaging with participants and their families to create and promote stimulating in-home recreational activities, thereby nurturing a supportive and enriching atmosphere that enhances participants' overall quality of life.

Key Responsibilities

  • Provide training and mentorship to center staff to ensure daily operations align with DHCS and CMS standards.
  • Ensure timely submission of all required information to DHCS and CMS, verifying the accuracy of all HPMS submissions.
  • Communicate with DHCS and CMS regarding HPMS quality reporting and corrective action plans.
  • Develop and implement various PACE Committees, including but not limited to the Medical Advisory Committee and Quality Improvement Committee.
  • Coordinate with the center manager to conduct annual participant satisfaction surveys.
  • Oversee compliance with all PACE regulations and continuously provide training and updates to the day center and clinic teams.
  • Track and report all PACE data within HPMS.
  • Investigate and document any security incidents.
  • Collaborate with the center manager to report all SDR, grievances, and appeals in HPMS.
  • Attend monthly and quarterly meetings, preparing reports for DHCS and CMS.
  • Design and implement audit training, preparing the team for audits.
  • Safeguard the privacy and confidentiality of associates, participants, and sensitive agency information.
  • Perform other duties as assigned.

Qualifications

Education:

Bachelor's degree in healthcare or a related field.

Experience:

  • Experience in data management and analysis.
  • Experience working with diverse populations and cultures.
  • Minimum of one (1) year of experience with the frail elderly demographic.

Skills and Knowledge:

  • Strong written and verbal communication abilities.
  • Proven problem-solving skills and sound judgment.
  • Ability to work independently and effectively.
  • Proficient in workplace computer skills or willingness to learn.
  • Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

Additional Requirements:

  • Physical capability to perform job duties.
  • Routine travel between PACE sites and within the community is required.
  • A valid driver's license is necessary.

Physical Demands:

  • Must be able to remain stationary for 50% of the time.
  • Occasional movement within the office to access files and equipment.
  • Ability to operate a computer and other office machinery.
  • Capable of maintaining files in cabinets, which may involve reaching, kneeling, or crouching.
  • Effective communication and information exchange are essential for this role.
