Patient Care Coordinator

Position Purpose
The primary aim of this role is to perform comprehensive evaluations of participants in the PACE (Program of All-Inclusive Care for the Elderly) initiative, working in close collaboration with other members of the Interdisciplinary Team (IDT) to develop and execute personalized care strategies. This position is crucial in creating a variety of activities and exercise regimens designed to improve the physical and emotional health of participants. Furthermore, the role entails engaging with participants and their families to craft and promote stimulating in-home recreational activities, thereby nurturing a supportive and enriching atmosphere that enhances the overall quality of life for participants.
Key Responsibilities
- Oversees the medical care of participants within the designated program, clinic, or service.
- Competently executes clinical skills and modifies interventions in response to changes in participant or family circumstances.
- Establishes and implements a follow-up system for participants regarding missed appointments, abnormal test results, and required services.
- Provides timely updates and relevant information to referring healthcare providers as needed.
- Organizes and coordinates appointments and services with participants, their families, and relevant providers.
- When necessary, secures required authorizations from third-party payers for specialized care, diagnostics, durable medical equipment, and community services.
- Monitors clinic access to ensure new participants receive timely attention in accordance with established standards.
- Adjusts scheduling priorities for participants as needed.
- Facilitates participant flow during clinic operations and delegates tasks appropriately.
- Collaborates with physicians and management to formulate long-term plans and objectives for the assigned area, including recommendations for enhancing clinic efficiency.
- Sets goals and strategies to achieve the plans and objectives for the assigned area.
- Communicates effectively with physician and provider partners.
- Offers support and education to nursing staff regarding current treatments and clinical programs.
- Conducts participant check-ins at the clinic, records vital signs, and prepares charts for physician review.
- Performs other duties as assigned.
Qualifications
Education:
- Certified Nursing Assistant certification.
- Current BLS/First Aid certification required.
Experience:
- A minimum of two years of experience working with frail or elderly populations.
Skills and Knowledge:
- Strong written and verbal communication abilities.
- Proven problem-solving skills and sound judgment.
- Ability to work independently.
- Proficient in computer skills or willingness to be trained.
- Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.
Other Requirements:
- Physical strength to perform job duties.
- Routine travel between PACE sites and within the community is required.
- Valid driver's license is necessary.
Physical Demands:
- Must be able to remain stationary for 50% of the time.
- Occasional movement within the office to access files and equipment.
- Able to operate a computer and other office machinery.
- Ability to position oneself to maintain files in cabinets.
- Effective communication skills are essential for this role.
