Multilingual Support Coordinator

Hudson Guild is in search of a dedicated Multilingual Support Coordinator for our Naturally Occurring Retirement Community (NORC) initiative. We seek an individual who is organized, compassionate, and values diversity in all its forms, including culture, ethnicity, and sexual orientation.

Role Overview:
  • Deliver a variety of services, including direct assistance with benefits and entitlements, case management, group programming, advocacy, resources, and referrals.
  • Conduct initial assessments, screenings, and evaluations as assigned.
  • Lead support groups as designated by the supervisor.
  • Assist participants with their case management and support needs.
  • Provide information and facilitate connections to other relevant services.
  • Maintain and complete timely records for all participants in the 'STARS' Peerplace database, including progress notes, case interactions, and summaries, while generating monthly reports.
  • Adhere to all funding agency service standards and contractual obligations.
  • Engage in case presentations, staff meetings, field visits, and ongoing training.
  • Submit work schedules and performance statistics promptly as required.
  • Keep the supervisor informed of ongoing priorities and updates during supervisory meetings.
  • Participate in community-building initiatives sponsored by the program and encourage participant involvement.
  • Be mindful of programmatic objectives and strive to meet these in all participant interactions.
  • Engage in professional development and apply acquired knowledge to implement evidence-based best practices.
  • Participate in data analysis processes conducted by the Evaluation and Quality team to identify strengths and areas for enhancement, following action steps in the Continuous Quality Improvement (CQI) framework.
  • Foster cooperative and supportive relationships with Hartley House, Hudson Guild, and associated programs and staff.
  • Perform additional tasks as assigned.
Work Environment:

The position is typically office-based but requires travel between the office and clients' homes, as well as potentially multiple sites within a community center setting.

Physical Demands:

This role necessitates frequent communication with participants, management, and colleagues. The individual must be able to remain stationary, operate a computer and other office equipment, such as telephones and copiers, and occasionally lift items weighing up to 20 lbs.
