Care Coordinator

Care Design New York is seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will play a vital role in helping individuals with intellectual and/or developmental disabilities achieve their long-term and short-term goals.
Key Responsibilities- Advocate for and with our members to ensure informed decision making, informed consent, and appropriate guardianship.
- Schedule, lead, and collaborate with members and their interdisciplinary teams to conduct meetings and assessments, ensuring the development of comprehensive, person-centered Life Plans that reflect the person's needs and desired life goals.
- Implement, update, and monitor Life Plans, and facilitate individualized Life Plan reviews and approval processes.
- Ensure integration of all needed and preferred supports and services, including medical, behavioral, social, habilitation, dental, psychosocial, and community-based and facility-based long-term supports and services.
- Maintain ongoing contact with critical people in a member's life.
- Ensure timely submission of all documentation, including Life Plans, Progress notes, etc.
- Assist members in maintaining benefits such as Social Security, Supplemental Security Income, Medicaid, and Medicare coverage, and Food Stamps.
- Assist members in resolving problems in living, such as housing, utilities, the judicial system, and general safety.
- Report abuse or neglect immediately when observed or reported.
- A Bachelor's degree with 2+ years of relevant experience, OR
- A License as a Registered Nurse with 2+ years of relevant experience, which can include any employment experience and is not limited to case management/service coordination duties, OR
- A Master's degree with 1+ year of relevant experience.
- Bilingual in English and Mandarin.
- Salary starting at: $31.05/hour
- Salary up to: $33.49/hour
- Generous benefits, including health, vision, and dental insurance, paid time off, $3000 tuition reimbursement per calendar year, and up to $250 of professional development courses.
- Qualifying employer under the Public Service Loan Forgiveness (PSLF) program.
