Care Coordinator

At Independence Care System, we are seeking a highly skilled Care Navigator to join our Health Home team. As a Care Navigator, you will play a critical role in coordinating members' overall care, ensuring they receive the necessary health and social supports to maintain their health, mobility, and independence in the community.
Key Responsibilities:- Coordinate Care: Work with members, Nurse Care Managers, and other departments to ensure proper handling of member requests, inquiries, and concerns.
- Develop and Implement Plans of Care: Create disability-competent, member-centered plans of care that address common barriers for people with physical disabilities.
- Monitor and Evaluate Health and Wellness Needs: Identify and address health and wellness needs, including preventative and specialty care, through the creation of a disability-competent member-centered plan of care.
- Collaborate with Members and Care Teams: Work in collaboration with members and the Nurse Care Manager to develop and implement members' plans of care, including the creation of SMART member-stated wellness and recovery goals.
- Coordinate Access to Services and Supports: Facilitate communication and provide coordination of care between all health and community providers to ensure members receive the necessary services and supports.
- Education and Experience: Bachelor's Degree and at least 1 year of full-time, relevant work experience.
- Problem-Solving Skills: Demonstrated problem-solving skills and ability to work efficiently with all levels of colleagues, clients, and external contacts.
- Technical Skills: Proficient in MS Word, Excel, and Outlook.
- PTO and Holidays: 28 days of PTO, plus 11 holidays, in your first year.
- Hybrid Work Schedule: 35 hour work week with approximately 40% remote work.
