Role Overview:

The Community Engagement Specialist (CES) plays a pivotal role in fostering connections within the community to attract Medicare seniors to Oak Street Health. The CES is tasked with engaging seniors to encourage them to seek care from Oak Street Health.

In collaboration with Outreach Managers, Outreach Directors, and Community Relationship Managers, the CES will focus on driving local patient growth within their designated area.

Key Responsibilities:

  • Lead Generation: Cultivating potential leads by effectively interacting with the Medicare senior demographic through community events and outreach initiatives. Staying informed on internal sales systems and marketing strategies by participating in regular meetings to learn about updates on services and initiatives. Conducting prospecting and outreach to new leads assigned to you, which may include leads generated through digital channels or referrals from other Community Engagement Specialists. Engaging prospective patients through various communication methods including email, phone, and networking opportunities. Collaborating with Community Relationship Managers, fellow CESs, and Outreach Managers/Directors to enhance lead generation efforts with local community members or Insurance Agents.
  • Relationship Development: Maintaining regular communication with prospects to foster relationships that drive patient growth in the assigned area. Documenting customer interactions and sales activities in the customer relationship management (CRM) system and other relevant business platforms. Serving as a liaison for both the clinical and center teams to address any inquiries regarding patients. Addressing any concerns or complaints from new prospects and patients about Oak Street Health by understanding their issues or directing them to appropriate staff members. Performing additional duties as required.

  • Skilled in navigating external challenges to ensure a positive experience.
  • Capable of managing multiple priorities effectively.
  • A collaborative team player contributing to overall team success.
  • Experience in sales and/or healthcare is advantageous.
  • Proficiency in Spanish, Polish, Russian, or other languages relevant to the communities served (as needed).
  • Authorization to work in the United States.
