Care Navigator Social Support Services

Job Summary:

We are seeking a highly motivated and compassionate Care Navigator to join our team at The Institute for Family Health. As a Care Navigator, you will play a vital role in supporting patients with their personal and environmental difficulties that may impact treatment adherence.

Key Responsibilities:

  • Assist patients in addressing personal and environmental difficulties that may negatively affect treatment adherence
  • Ensure the execution of service plans by accompanying patients to medical appointments, promoting healthy living, and providing social support
  • Conduct home visits and community outreach to engage patients and obtain health home consents
  • Provide case management to address patients' individual needs related to treatment adherence, including transportation to medical appointments and childcare

Requirements:

  • HS Diploma or GED required
  • Knowledge of programs and services
  • Ability to write professional reports and correspondence
  • Ability to work independently and as part of a team
  • Knowledge of healthcare
  • Excellent interpersonal and time management skills
  • Excellent computer skills required
  • Bi-lingual, Spanish preferred
