Job Summary

Professional Care Medical Practice, P.C. is seeking a highly skilled and dedicated Licensed Mental Health Counselor or Licensed Social Worker to fill the role of Behavioral Health Professional in our medical department at the Ulster County Jail in Kingston, NY.

Job Responsibilities
  • Conduct comprehensive clinical assessments and develop treatment plans for inmates/patients, including documentation of findings and referrals to psychiatry when necessary.
  • Perform risk assessments using structured interviews and risk assessment tools, and manage inmates at risk for suicide and self-harm according to established protocols.
  • Serve as the mental health liaison between the medical unit and facility staff, facilitating communication and coordination of services.
  • Collaborate with other healthcare professionals to ensure high-quality mental health services and adherence to NCCHC standards.
  • Develop and conduct in-service education programs to enhance staff knowledge and skills.
  • Collect and analyze data to prepare monthly reports on performance and quality of care.
Requirements
  • Licensure as a LMHC, LMSW, or LCSW in the state of New York.
  • Master's degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical coursework, training, and/or experience.
  • Minimum of two years of experience and familiarity with substance abuse/MAT counseling preferred.
  • Valid driver's license and reliable transportation.
