The Pharmaceutical Care Specialist is responsible for maintaining the integrity, safety, and efficacy of the medication management process for all patients within the designated specialty or geographic area, which includes diverse patient demographics: Neonates, Pediatrics, Adolescents, Adults, and Geriatrics.

Licensure, Certification, or Registration Requirements for Hire:
Current licensure with the Virginia State Board of Pharmacy is required or must be obtained within the initial 90 days of employment.

Licensure, Certification, or Registration Requirements for Continued Employment:
Must maintain a valid Pharmacist license with the Virginia Board of Pharmacy.

Experience:
REQUIRED:
N/A

PREFERRED:
At least one (1) year of pharmacy practice experience in a comparable environment or completion of residency training.
Prior experience with advanced drug distribution and clinical service methodologies is advantageous.

Education/Training:
REQUIRED:
Bachelor of Science in Pharmacy from an accredited institution.

PREFERRED:
Completion of a PGY1, ASHP-accredited residency
Completion of a PGY2, ASHP-accredited residency
Doctor of Pharmacy (Pharm.D.) from an accredited institution.

Independent Responsibilities:


Oversee the monitoring, screening, and verification of all medication-related orders to ensure appropriate dosage, route, dosage form, and administration schedule.

Facilitate the timely preparation and dispensing of all dosage forms by pharmacy staff.
Address and resolve medication therapy issues.
Clarify and amend problematic medication orders.
Assign specific tasks to pharmacy technicians. Evaluate the performance of employees and students.
Provide counseling to patients regarding the safe and effective use of their medications.
Resolve equipment and operational challenges. Collaborate with other healthcare professionals in a multidisciplinary setting.
Monitor medication effects and recommend or initiate necessary adjustments to achieve optimal medication-related outcomes.

Supervisory Responsibilities (if applicable):


Supervise the activities of Pharmacy Interns and Certified Pharmacy Technicians in accordance with the Pharmacy Act and the Drug Control Act, along with related statutes and regulations of the Virginia Board of Pharmacy.

Oversee the actions of pharmacy residents as outlined by the Department of Pharmacy Services.

Additional Position Requirements:
Availability to work evenings, nights, and weekends is essential.
