Your Responsibilities

As a Clinical Care Specialist at Honest, you will play a pivotal role in ensuring exceptional clinical care for your patients. You will utilize an integrated technology platform and collaborate with a diverse team of professionals, including registered nurses, nurse care managers, health coaches, dieticians, pharmacists, psychiatrists, social workers, and specialists – all within the Honest network.

Key Responsibilities Include:
  • Deliver comprehensive management of patient conditions, encompassing assessment, diagnosis, treatment, and ongoing consultation.
  • Facilitate transitions of care through collaborative pathways with post-acute care facilities.
  • Accurately document patient conditions and interactions in an electronic medical record (EMR).
  • Ensure high levels of quality and patient satisfaction with every interaction.
  • Collaborate with Honest's care team to identify and address social determinants of health affecting your patients, including functional capacity, home environment, health literacy, housing instability, food insecurity, and caregiver support.
  • Perform additional related duties as assigned.
  • Options for Full-Time or Part-Time availability.
Qualifications

  • Must have reliable access to high-speed internet to facilitate seamless remote work communication and productivity.
  • You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C).
  • You possess an unrestricted Nurse Practitioner or Physician Assistant state license, including board certification, or the ability to achieve board certification within two years.
  • You have a strong understanding of clinical standards of care and are dedicated to enhancing the quality of life for your patients.
  • You are able to travel consistently to various work sites and maintain a valid driver's license and motor vehicle insurance.
