As a Clinical Care Specialist at Honest, you will play a pivotal role in ensuring the highest standard of clinical care for your patients. You will utilize an integrated technology platform and collaborate with a dedicated team of professionals, including nurse care managers, social workers, care coordinators, and transition of care coordinators—all part of the Honest family.

This position is remote, but you must be situated within or near our operational market to ensure availability for in-home patient care and to attend necessary meetings with team members and partners.

Key Responsibilities Include:
  • Deliver comprehensive management of patient conditions, encompassing assessment, diagnosis, treatment, and consultation over time.
  • Facilitate transitions of care through collaborative pathways with post-acute care facilities.
  • Accurately document patient conditions and interactions in an electronic medical record (EMR).
  • Ensure high levels of quality and patient satisfaction with each interaction.
  • Collaborate with Honest's care team to identify and address social determinants of health for your patients, including functional capacity, home environment, health literacy, housing instability, food insecurity, and caregiver support.
  • Perform additional related duties as assigned.
Qualifications

If you identified with the 'About You' section of this job posting, you are likely a great fit for our innovative culture.

  • You have completed an accredited program for Nurse Practitioner (NP) or Physician Assistant (PA-C).
  • You hold an unrestricted Nurse Practitioner or Physician Assistant state license, including board certification, or have the ability to become board certified within two years.
  • BLS certification is required.
  • Phlebotomy experience is preferred.
  • Experience in medication administration is preferred.
  • You possess a strong understanding of clinical standards of care and are focused on enhancing the quality of life for your patients.
  • You are able to travel consistently to various work sites and maintain a valid driver's license and motor vehicle insurance.

The base salary range for this role is $115,800 - $136,000. Compensation considers various factors, including a candidate's experience, education, skills, licensure, certifications, and organizational needs. Base salary is just one component of the comprehensive rewards program offered by Honest. Eligible roles may also qualify for short-term incentives and a robust benefits package.

Support and Benefits
  • As a full-time team member, you will benefit from Honest's exceptional total rewards package, including competitive base pay with bonuses, paid time off starting at four weeks for full-time employees, 12 paid holidays per year, reimbursement for continuing medical education, a 401k with match, and health, dental, and vision insurance.
  • Family-friendly policies that support paid parental leave and flexible work arrangements.
  • As a team member, you will be supported by our strong commitment to training and development that begins with onboarding and continues throughout your career with Honest.
  • You will collaborate with like-minded healthcare professionals who share your understanding of the importance and value of Honest's high-quality, value-based care model.
