RN Patient Care Coordinator - Cardiology Job Description

We are seeking a dedicated RN to join our team as a Patient Care Coordinator in Cardiology. In this role, you will play a vital part in ensuring seamless transitions between care settings and facilitating positive patient outcomes.

About the Position:

  • Act as a resource to interdisciplinary teams, patients, and families throughout the episode of care.
  • Schedule appointments, provide education, and address psychosocial needs to ensure effective care coordination.
  • Participate in multidisciplinary team meetings to discuss current and future patient cases.

Qualifications:

  • 1+ years of Case Management Experience or 1+ years of Patient Care Experience.
  • Bachelor's degree in Nursing preferred.
