We are seeking an experienced Intensive Care Transition Specialist to join our team at TotalMed. As a Critical Care Transition Coordinator, you will provide specialized care for patients transitioning from intensive care to general medical-surgical units.

About the Role

The Intensive Care Transition Specialist will be responsible for assessing, monitoring, and treating patients in the step-down unit. They will work closely with the interdisciplinary team to ensure optimal outcomes for patients.

Responsibilities
  • Provide specialized nursing care to patients transitioning from intensive care to general medical-surgical units.
  • Monitor and assess patients' vital signs, cardiac rhythms, and respiratory status in the step-down unit.
  • Administer medications, perform procedures, and collaborate with interdisciplinary teams to ensure comprehensive patient care.
  • Educate patients and families on post-intensive care recovery, medication management, and self-care techniques.
Qualifications

To be successful in this role, you will need:

  • A current RN license.
  • BLS certification (other certifications may be required by the facility).
  • Two years of recent experience in a Step Down Unit setting.
  • Strong communication and adaptability skills.
