Job OverviewWe are urgently hiring a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to work with our team. As a Patient Care Specialist, you will play a critical role in delivering high-quality patient care and making a meaningful difference in people's lives.
Salary and BenefitsThe estimated annual salary for this position is $65,000 - $85,000, depending on experience and qualifications. In addition to a competitive salary, we offer a range of benefits, including:

As a Patient Care Specialist, you will be responsible for:
- Participating in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and following the nursing care plan for each patient
- Accurately reporting and recording the patient's condition and care, including signs and symptoms which may be indicative of change
- Preparing clinical/progress notes and summaries
- Assisting patients with activities of daily living while teaching appropriate self-care techniques
- Providing and maintaining a safe environment for patients
- Following universal precautions whenever giving any aspect of patient care
- Following emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury or significant change in the patient's condition
- Supporting the CARE needs of both clients and care professionals
- Conducting quality assurance visits to ensure proper care delivery
- Performing new client/care professional introductions
- Participating in Care Professional orientation and training
- Contributing to company culture and living our core value of C.A.R.E.
To be successful in this role, you will need:
- A current state license as an LPN/RN
- Excellent interpersonal skills
- A valid driver's license and reliable transportation
- The ability to work a flexible schedule and travel
- Comfortable working in a fast-paced environment
- The ability to successfully pass a criminal background check, Motor Vehicle Records check, and drug screening
