Key Responsibilities

As the primary Service Coordinator, you will manage a caseload of consumers enrolled in Home and Community-based Services and/or Texas Home Living waiver programs. Your responsibilities will include:
  • Developing and implementing a Person Directed Plan (PDP) tailored to the individual needs and preferences of each consumer.
  • Facilitating planning meetings to identify consumer needs and ensuring ongoing evaluation of the PDP.
  • Monitoring service delivery and assessing consumer progress while coordinating with various internal and external service providers.
  • Maintaining accurate documentation and adhering to deadlines set by the unit and relevant regulatory bodies.
  • Driving to multiple service counties to conduct face-to-face meetings and attend planning discussions.
  • Serving as a liaison between clients, their families, and service providers to facilitate access to necessary support and treatment.

  • Effective communication skills, both verbal and written, to foster professional relationships.
  • Experience in assessing consumer needs and developing tailored service plans.
  • Proficiency in utilizing computer software for documentation and reporting purposes.
  • Ability to work independently while also collaborating effectively within a team environment.
