The Service Coordination Specialist serves as the primary liaison for individuals receiving support through Home and Community-based Services and Texas Home Living waiver programs. This role is crucial in ensuring that each consumer's needs and preferences are effectively addressed through tailored service plans.
Key Responsibilities- Assessment: Conduct thorough evaluations of consumer needs using appropriate assessment tools. Regularly update assessments to reflect any changes in status.
- Service Planning: Develop and continuously refine the Person Directed Plan (PDP) in collaboration with consumers and their families. Facilitate planning meetings to ensure all voices are heard and considered.
- Monitoring: Maintain regular face-to-face contact with consumers to ensure the effectiveness of the service plan. Document progress and satisfaction consistently.
- Documentation: Ensure all service tickets and progress notes are accurately entered into the Electronic Health Record (EHR) within specified deadlines.
- Communication: Maintain open lines of communication with consumers, families, and service providers. Respond to inquiries promptly and professionally.
Applicants should possess a Bachelor’s degree in psychology, social work, or a related field, or an Associate’s degree with relevant experience. Strong interpersonal skills and the ability to work collaboratively within a team are essential.
