Position Overview
As a key member of our organization, the Service Coordinator will take on the essential role of managing a caseload of clients within the Home and Community-based Services and Texas Home Living waiver programs. This position requires a strong commitment to developing and evaluating personalized service plans that align with the unique needs and aspirations of each consumer.

Key Responsibilities
  • Client Assessment: Conduct thorough assessments to identify consumer needs, utilizing appropriate tools and methodologies.
  • Service Planning: Create and maintain a Person Directed Plan (PDP) that reflects the consumer's preferences and goals, facilitating planning meetings to ensure collaborative input.
  • Monitoring and Evaluation: Regularly monitor the implementation of service plans, ensuring compliance with established goals and making necessary adjustments based on consumer feedback.
  • Documentation: Maintain accurate and timely records of service coordination activities, including progress notes and service tickets, in accordance with organizational standards.
  • Communication: Foster effective communication with consumers, families, and service providers, ensuring all parties are informed and engaged in the service delivery process.

Qualifications
The ideal candidate will possess a Bachelor’s degree in psychology, social work, or a related field, or an Associate’s degree with relevant experience in social services. A strong understanding of trauma-informed care principles and the ability to work collaboratively within a team environment are essential.
