About the Role

We are seeking a Senior Case Manager RN to oversee the provision of care for clients while maintaining compliance with agency policy and state regulations. This role involves creating individualized care plans, managing a team of RN Case Managers, and ensuring quality care in home settings.

Key Responsibilities

  • Create direct care and case management plans for assigned clients
  • Collaborate with regional directors and other personnel to identify and correct issues or improve services
  • Plan, implement, and evaluate care provided
  • Serve as a local on-site clinical resource and provide support to ensure client's home care needs are met

Requirements

To be successful in this role, you must have:

  • A graduate degree from a state-approved school of professional registered nursing
  • An active, unrestricted RN license in the state(s) of practice
  • A minimum of two years' experience in nursing, including one year in home care or a closely related field
  • One year of supervisory and/or case management experience preferred
